Application Process for Kindergarten & Grades 1-4

The following information is needed at the time of registration:

• Birth Certificate (if not Catholic)
• Baptismal Certificate
• Immunization Record
• Child's Social Security Number
• Progress Report (K)
• Copy of Readiness Test Results (Grade 1)
• Copy of First Penance and First Communion Certificates (Grades 3 & 4)
• Copy of most recent standardized test scores (Grades 2-4, if available)
• Copy of most recent report card (Grades 1-4)
• $50.00 non-refundable application fee per child

Please submit copies, not originals, as we will need these documents for our file

FINAL acceptance at Immaculate Conception School depends upon the review of records from previous schools. An application will not be considered complete until ALL the necessary paperwork has been submitted, including immunization records.

Also, no child shall be admitted to school without proper proof that the child has been immunized as required by the laws of the State of New Jersey, N.J.S.A 26:1A-9. Proper immunization is a condition of admission to the school.

Immaculate Conception School will verify parish registration.
