Medical Forms for the 2011-2012 School Year
As part of registration into Immaculata, ALL incoming freshmen are required to have a complete physical dated AFTER June 1, 2011, and all medical forms must be received before August 5, 2011.
** For returning students, if your child plans to participate in ANY sport or cheerleading during the next year, please take note of the following information:
- Your child must have a physical exam by your physician AFTER June 1, 2011
- Our Health Form package must be completed and dated AFTER June 1, 2011
- Our Health Form package with original signatures will be the only forms that can be accepted
- ALL Heath Forms must be received by the Athletic Trainer BEFORE August 5, 2011
- Physicals performed and/or dated prior to June 1st will not be accepted and your child will not be cleared for athletics for the coming school year
- We will personally hand these forms to students at the end of May. After that date, extra packages of Health Forms may be available at school during summer hours.
