Health Form Packages
** As part of registration into Immaculata, ALL incoming freshmen are required to have a complete physical dated AFTER June 1, 2013, and all medical forms must be received before August 2, 2013.
** For returning students, if your child plans to participate in ANY sport or cheerleading during the next year, please take note of the following information:
Your child must have a physical exam by your physician AFTER June 1, 2013
Our Health Form package must be completed and dated AFTER June 1, 2013
Our Health Form package with original signatures will be the only forms that can be accepted
ALL Heath Forms must be received by the Athletic Trainer BEFORE August 2, 2013
Physicals performed and/or dated prior to June 1st will not be accepted and your child will not be cleared for athletics for the coming school year
We will personally hand these forms to students at the end of May. After that date, extra packages of Health Forms may be available at school during summer hours.
